Patient Care Navigator

The Patient Care Navigator role at Center for Elders' Independence involves providing administrative support for the HCBA Case Management Team, interfacing with participants, and handling various tasks such as data collection, scheduling, and care coordination. The position requires excellent customer service and oral communication skills, as well as experience working with electronic medical records and intermediate/advanced computer skills.

Requirements

• Bachelor's degree in social work, gerontology, marriage and family therapy, or psychology (highly preferred)
• Demonstrated excellent customer service and oral communication skills
• Health plan experience with member coordination/communication and clerical management experience (usually 3-5 years) preferred
• Experience working with electronic medical records and intermediate/advanced computer skills (Microsoft Office Suite) required
