Job Description
The IRIS Consultant provides ongoing support and assistance to participants in the IRIS program, assisting them in self-directing services to meet their long-term care goals and life outcomes.
Requirements
- High school diploma or equivalent
- Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (preferred)
- Four years of experience related to the delivery of social services (with high school degree or equivalent)
- One year of experience (with bachelor's degree)
- Valid driver's license
- Vehicle liability insurance
- Ability to travel in a three-hour radius
- Flexibility and willingness to shift priorities
- Strong interpersonal and communication skills
- Ability to use laptop computer, cell phone, and other mobile equipment
