Case Manager

Centerstone is hiring Case Managers to support mental health and substance use programs in Sarasota and Manatee counties. Case Managers provide wraparound services, outreach, and care coordination to individuals with mental health and/or substance use disorders.

Requirements

• Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor
• Provides individual and group mental health and substance use disorder case management services
• Develops, implements, evaluates, and updates treatment plans with input from the client and family
• Conducts clinical behavioral health assessments
• Maintains departmental productivity expectations
