Case Manager

Centerstone is seeking a Case Manager to provide mental health, substance abuse and outreach services to individuals with mental health and/or substance use disorder. The Case Manager will work with community partners and local stakeholders to provide comprehensive care and link clients and their families to needed resources.

Requirements

• Bachelor's degree required
• Valid FL driver's license
• Good organizational and time management skills
• Competency with electronic medical records and computer systems
• Strong interpersonal communication skills
