Job Description
Case Manager, Clinical Services. Coordinate services and care for patients, provide coaching, strategies and coordination of health and wellness resources, and teach the importance of the connection between physical and mental health. Flexible work settings.
Requirements
- Bachelor's degree in counseling, psychology, social work or related field
- Valid driver's license in good standing
- Experience with Electronic Health Record preferred
