Job Description
Centerstone is seeking a Case Manager to coordinate services and care for patients in various settings, providing coaching, strategies, and resources for health and wellness. The ideal candidate has a bachelor's degree in counseling, psychology, social work, or a related field and is passionate about making a difference in the lives of people with mental health issues.
Requirements
- Bachelor's degree in psychology, social work, or a related field
- Experience with Electronic Health Record preferred
- Valid driver's license in good standing required
- Ability to lift occasional
- Ability to sit for 90%
- Ability to stand for 10%
- Ability to squat, kneel, and bend occasionally
