Clinical Services Case Manager

We are seeking a Case Manager to coordinate services and care for patients, providing coaching and strategies to help them live their lives more fully. This is a great opportunity for individuals with a bachelor's degree in counseling, psychology, social work, or a related field to make a difference in the lives of people coping with mental health issues.

Requirements

• Help patients navigate the healthcare system through coaching, education, and support
• Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness
• Use behavioral management techniques, crisis intervention, active listening, and motivational interviewing to help patients develop better life management skills
