Job Description
As a CenterWell Home Health nurse, you will provide personalized care to patients in their homes, working with a dedicated team to manage care plans and help patients regain independence. You will implement individualized care plans, educate patients and their families, and maintain accurate records of patient care.
Requirements
- Current nursing license in the practicing state
- Valid drivers license, auto insurance, and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
