Job Description
As a CenterWell Home Health nurse, you will provide personalized, one-on-one care to patients in their homes, helping them regain independence and recover from injury, illness, or surgery. You will work closely with a dedicated team of physicians and clinicians to develop and manage care plans, provide hands-on care, and educate patients and their families on proper care and management.
Requirements
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Strong med surg, ICU, ER, acute experience
- Home Health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver’s license, auto insurance and reliable transportation
