The Caseworker 2 FACT Team position involves performing moderately complex combination of team led, evaluation and case management duties as directed by the FACT Team Lead, and Program Manager. This role serves as a liaison between clients and the criminal justice system, providing intensive case management services to clients in lieu of incarceration. The position requires frequent travel and interaction with clients, families, law enforcement officials, and other community stakeholders. The caseworker assesses consumers regarding FACT eligibility, interviews individuals and families, and identifies psychosocial, economic, and physical needs of clients. The role also involves providing crisis intervention services, documenting case records, and participating in training and time studies.
