Job Description
The Primary Care Physician provides comprehensive medical care to eligible veterans in an outpatient setting, following VA and national clinical standards.
Requirements
- Provide comprehensive primary care services (preventive, acute, chronic)
- Diagnose and treat conditions such as diabetes, hypertension, COPD, heart failure
- Manage patient panels and follow-up care
- Conduct telehealth visits and virtual care
- Lead Patient Aligned Care Team (PACT)
- Coordinate with specialists, pharmacists, social workers, and community care
- Maintain accurate records in VA Electronic Health Record (EHR)
- Complete documentation and coding after each encounter
- Follow HIPAA, VA policies, and clinical guidelines
