Bilingual Care Coordinator - Behavioral Health

We are seeking a Bilingual Care Coordinator to serve as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff, and assists patients in navigating referrals to community resources and specialty care.

Requirements

• Bachelor's in Psychology, Social Work, Counseling or related degree.
• 5 years of experience can be substituted for a degree.
• Collaborative, and client centered.
• Organization is necessary to managing client information, appointments and follow up items.
• Collaborative team member who is ready to jump in, support our clients and team.
