Job Description
The Wraparound Care Coordinator provides home-based services to assist youth and families within the Erie County System of Care and who are receiving services through Erie County Department of Social Services. The role involves strength-based, individualized service planning, linkages, coordination, and monitoring of services to improve and promote family stability.
Requirements
- Bachelor’s Degree or Master’s Degree in a human services related field
- 1 year experience in a professional or internship human services setting
- Valid NYS Driver’s License and adequate auto insurance
- Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds
