Job Description
The Care Coordinator II role at Hillside's Youth Assertive Community Treatment (Youth ACT) program in Rochester, NY, focuses on coordinating the psychosocial care of children/youth with serious emotional disturbance, and their families. This position involves assessment, development of plan, training, and providing support within a multi-disciplinary team, working with referrals from C-SPOA. The role offers flexible hours and occasional on-call responsibilities for youth program support.
Requirements
- CPR certification or obtained within 60 days of hire
- Valid NYS driver’s license with clean driving record and insurance coverage
- Knowledge of federal, state, and local statutes
- Ability to de-escalate and manage crisis situations
- Ability to deliver psychoeducational training
- Ability to work collaboratively as part of a multi-disciplinary treatment team
