The role involves welcoming clients, building rapport, coordinating care, and acting as a liaison between clients and internal teams to ensure client needs are met. Requirements

• Place outbound calls, emails and texts to admitted clients.
• Reach out to clients to ensure they fully enrolled in their treatment plan.
• Act as a liaison between patients and internal Charlie Health teams.
• Support client-related requests from the Clinical Care team.
• Manage client schedule and rescheduling appointments.
• Complete all documentation in a timely manner.
• Adapt to organizational change and departmental restructuring.
