Discharge Planner

We are looking for a Discharge Planner to help manage client-related communication and coordination with referral sources. The role involves providing treatment updates to referring providers, developing discharge plans, and identifying aftercare resources for clients discharging from Charlie Health. The ideal candidate is passionate about our mission to connect the world to life-saving mental health treatment and is inspired by the opportunity to impact thousands of lives in a meaningful way.

Requirements

• Bachelor’s degree in health sciences, communications, psychology, social work, or related field
• Minimum of 2 years of relevant work experience
• Strong interpersonal, relationship-building and listening skills
• Metrics- and results-oriented mindset
• Excellent written and verbal communication skills
• Extreme organization and attention to detail
• Work authorized in the United States and native or bilingual English proficiency
• Ability to thrive in a fast-paced environment and learn quickly
• Proficient in Salesforce and Google Suite/MS Office
