Cheer Home Care is hiring a Care Case Manager in San Diego. The Care Case Manager creates the unique care plan, manages the care, and provides constant feedback to clients, clients representatives and caregivers.

Requirements

• Upbeat Personality
• Master in Social Work, Nursing or Relevant Field, preferred
• Industry Experience, preferred
• Strong Computer Skills
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance (LIC 9163)
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call
