Care Manager Physical Therapy Home Health

We are looking for a PT Case Manager to collaborate with an interdisciplinary team to develop and implement individualized care plans and provide constant feedback to clients and healthcare providers. The ideal candidate will have experience as a Physical Therapist, excellent communication and problem-solving skills, and a strong organizational mindset.

Requirements

• Active California Physical Therapist License
• 3 years of experience as a Physical Therapist
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Valid California drivers license and car insurance
• Willing to work one weekend a month as back-up on call
• Ability to multitask, drive, take action
• Strong Organizational & Communication Skills
• Cell phone with internet access
