Care Manager Physical Therapy Home Health

We are seeking a PT Case Manager to join our team at Cheer Home Care in La Jolla, California. As a PT Case Manager, you will collaborate with an interdisciplinary team to develop and implement individualized care plans, deliver interventions to improve mobility, strength, and independence, and work closely with our office team to provide excellent communication and problem-solving skills.

Requirements

• Active California Physical Therapist License
• 3 years of experience as a Physical Therapist
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Valid California drivers license and car insurance
• Willing to work one weekend a month as back-up on call
• Ability to multitask, drive, take action
• Strong Organizational & Communication Skills
• Cell phone with internet access
