Cheer Home Care is hiring a Care Case Manager in San Diego to create unique care plans, manage care, and provide feedback to clients and caregivers. The role requires an upbeat personality, master's degree in a relevant field, industry experience, strong computer and organizational skills, and the ability to work one weekend a month as back-up on call.

Requirements

• Master in Social Work, Nursing or Relevant Field, preferred
• Industry Experience, preferred
• Strong Computer Skills
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance (LIC 9163)
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call
