Care Coordinator

We're a unique primary care provider changing lives every day. We're rapidly expanding and seeking a Care Coordinator to join our team. This role involves authorizing, scheduling, and ensuring completion of patient visits with specialty care, as well as communicating with physicians, clinicians, and other medical personnel.

Requirements

• Serve as primary point of contact for incoming and outgoing patient referrals
• Facilitate communication, collaboration, and coordination of care
• Schedule patients, arrange transportation, and notify patients of appointment information
• Use web-based insurance platforms to generate referral authorizations
• Provide extraordinary customer service to all internal and external customers
