Primary Care Physician

We’re a unique primary care organization that's changing lives every day. We're looking for innovative and entrepreneurial-minded physicians to join our team as Primary Care Physicians (PCPs). Our PCPs will work with patients to build trust, provide evidence-based care, and champion a positive culture within our medical centers.

Requirements

• Provide care for patients with acute and chronic illnesses
• Accountability for patient outcomes and cost-effectiveness
• Act as a health coach to help patients set and achieve health goals
• Simplify and prioritize care to facilitate behavior change
• Collaborate with care teams to achieve high-quality outcomes and quality measures
• Develop and improve leadership skills to benefit patients, teams, and the company
