Family-Centered Therapist-Contract

Choices Coordinated Care Solutions is seeking an independent contractor to provide Therapeutic Services for children, youth and adults involved with DCS or Juvenile Probation. Services may be individual or family. The independent contractor will work alongside DCS and a multi-disciplinary team to provide clinical support.

Requirements

• Master's or Doctorate Degree in Human Services field such as Psychology or Social Work
• Licensed (or in possession of a temporary license) issued by the Indiana Behavioral Health & Human Services Licensing Board in one of the following: Licensed Social Worker (LSW), Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFTA/LMFT), Licensed Mental Health Counselor (LMHCA/LMHC)
• Valid driver's license and reliable vehicle for transportation
