Job Description
Case Manager I (MSW/RN) position for a Medicare/DSNP plan. Provides telephonic case management for members with acute, chronic, and complex needs. Required qualifications include a Bachelor's degree in nursing or a master's degree in social work, with a minimum of one year of case management experience.
Requirements
- Bachelor's degree in nursing or a master's degree in social work
- Current, unrestricted license in the State of Washington as a registered nurse (RN) or a Social Worker (LSWAA, LSWAIC)
- Minimum of one year case management experience
- Minimum of three years of clinical experience in an acute care and/or outpatient setting
- Experience with Microsoft Office products
- Preferred: Case Management Certification, Bilingual abilities
