The Community Health Worker serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Requirements
- Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness.
- High school diploma or equivalent.
- Certification as a Peer Specialist or Community Health Worker.
- Ability to drive and maintain an acceptable driving record.
