Community Health Worker (CHW)

Share is seeking a Community Health Worker (CHW) to serve as a frontline public health worker, facilitating access to health and social services, improving service delivery, and building individual and community capacity through education, support, and advocacy.

Requirements

• High school diploma or GED
• Certification as a Community Health Worker
• 1-2 years of experience in community health, outreach, or social services
• Knowledge of public health issues, health education, and community resources
• Strong interpersonal skills and ability to engage diverse populations
