Job Description
The Baltimore City Health Department seeks a Grant Service Specialist II Cardiovascular Disease Program Coordinator to work with the Office of Chronic Disease Prevention. The GSS II will collaboratively develop a standardized process to identify individuals at risk for hypertension, screen them for social support needs, and provide navigational support and assistance.
Requirements
- Have a degree from an accredited college or university.
- Have two years of experience giving presentations and facilitating discussions is required.
- Experience with motivational interviewing is preferred.
- Have experience working in the community/as a Community Health Worker is preferred.
