Case Manager

The Case Manager will collaborate with the individual and family to identify the outstanding educational, vocational, and aftercare needs required to support a successful re-entry to the youth’s home environment upon discharge. The Case Manager will also co-facilitate DBT practices in the milieu including mindfulness and skills groups, and support milieu staff with skill acquisition as needed.

Requirements

• Associate’s Degree required, Bachelor’s degree preferred
• Minimum 3-years’ experience in residential program, 5+ years’ preferred
• Lived experience and experience, training, and a history of working with adolescents – Preferred
• Valid driver’s license to utilize company vehicle - Required
