The Project HEAL Case Manager Position is a collaborative partnership between Claratel Behavioral Health and other community partners to support individuals transitioning from hospital and acute care settings into Transitional Housing. The Case Manager will provide a combination of community transition, planning, case management, and psychosocial rehabilitation services to ensure clients receive coordinated, wrap-around care that promotes stabilization, recovery, and long-term housing success.
