Job Description
The Outreach Case Manager position is an integral part of the Community Based Services Program within Claratel Behavioral Health. The role involves working with day shelter guests who may have behavioral health and/or substance use treatment needs to provide engagement, screening, care coordination, and linkage to ongoing treatment services.
Requirements
- High school diploma or GED
- At least one year of experience in a behavioral health setting providing case management/community support services
- Basic computer and typing skills
- Care coordination skills
- Transportation to and from work
