The Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients, ensuring patient progress and effective care.

Requirements

• Communicates and coordinates with team members to provide quality patient care
• Documents patient activities and medical necessity in the healthcare record
• Supports length of stay management and conducts utilization reviews
• Communicates with payors and ensures timely updates to the clinical team and administration
