Advanced Practice Provider (NP, PA, CNS) - Primary Care I Dover (Sign -On Bonus Eligible)

Cleveland Clinic is seeking an Advanced Practice Provider (NP, PA, CNS) to join the Primary Care team at Cleveland Clinic UPS Central Practice in Dover, Ohio. The APP will provide comprehensive primary care services to patients across the lifespan, delivering high-quality, evidence-based care focused on health promotion, disease prevention, and the diagnosis and management of acute and chronic conditions.

Requirements

• Conduct thorough patient assessments, develop and implement individualized treatment plans, prescribe medications, coordinate care across specialties, and educate patients
• Work closely with physicians, nursing staff, and interdisciplinary team members to ensure seamless continuity of care and outstanding patient experiences
• Maintain professional development and promote translation of evidence-based practices, guidelines, and research
• Participate in quality monitoring, data collection and analysis, and may conduct research studies
• Assist in onboarding new caregivers and may serve as a clinical preceptor for students
