Job Description
Join Cleveland Clinic Avon Hospital and experience world-class healthcare at its best. As a Case Management Social Worker, you will be responsible for coordinating the care and services of patients through comprehensive psychosocial assessments, supportive counseling and linkage to community resources. You will work days from 8:00 a.m. -- 4:30 p.m., with a weekend shift every three weeks.
Requirements
- Master’s degree in social work (MSW) or Master of Science in Social Administration (MSSA)
- Current state licensure as a Social Worker (LSW, LISW or LISW-S)
- Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross
- Valid Ohio Driver’s license upon hire
- Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire
