Job Description
Join Cleveland Clinic's Lodi Hospital as a Case Management Social Worker to coordinate patient care and services, conduct psychosocial assessments, and provide supportive counseling. This role involves intervention with patients experiencing complex psychosocial or discharge planning challenges, and facilitation of safe, effective transitions of care for high-risk patients.
Requirements
- Master’s degree in social work (MSW) or Master of Science in Social Administration (MSSA)
- Current state licensure as a Social Worker (LSW, LISW or LISW-S)
- Basic Life Support (BLS) certification through the American Heart Association (AHA) OR American Red Cross
