Requirements

• 1–3 years of experience in healthcare outreach, patient engagement, admissions, sales, or a comparable patient-facing role
• Experience with EMR systems, with familiarity with Athena preferred
• Prior experience in home health, hospice, managed care, or chronic condition programs preferred
