Job Description
Addiction and Mental Health Worker position at the HART Hub Oxford, Woodstock, ON. Support individuals experiencing withdrawal from substances in a community setting, provide coordinated, collaborative, community-based health and social services, and assist clients with integrated care planning, system connection and navigation, while providing onsite support with basic needs, mental health and addiction supports.
Requirements
- University degree or college diploma in a related field, such as social work, social service work, psychology, or addiction
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Valid G Drivers licence and access to a vehicle with at least $2 million in liability insurance to transport clients
- Demonstrated ability to attend work regularly
