Job Description
We are seeking a Transitional Case Manager to support individuals transitioning from hospital to independent living in the community. The role involves assessing client needs, designing rehabilitation plans, and providing skill teaching and support. The ideal candidate will have a diploma or degree in a health or social service discipline and experience working with individuals with serious mental illness and addiction.
Requirements
- Diploma or degree in a health, social service or a related discipline
- Valid driver's licence and access to a vehicle with adequate insurance
- Ability to attend work regularly
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), the College of Psychologists of Ontario (CPO), or the College of Nurses of Ontario (CNO)
- First Aid & CPR, ASIST, and CPI certification
- Bilingual both official languages, preferred
