The Resident Care Coordinator serves as a liaison between residents, their families, and the care team, promoting a supportive and safe living environment.

Requirements

• Schedule and assign caregiving and medication technician shifts
• Supervise, train, and support care team members
• Maintain accurate and up-to-date resident records
• Coordinate care planning with home health agencies
• Coordinate admissions with the sales team
• Facilitate effective communication among residents, their families, and healthcare providers
• Address residents' special needs, requests, and complaints
• Assist with medication administration and provide direct care to residents as needed
