Colorado Access is looking for a Youth Intensive Care Coordinator to help shape their vision and support their mission. The coordinator will coordinate care for high-acuity youth, develop and implement strengths-based care plans, and navigate complex systems to support integrated care.
Requirements
- Master’s degree in social work, human behavioral sciences field or another healthcare field and/or clinical license (e.g., RN, LCSW, LPC)
- Three years of clinical, care management and/or healthcare experience required
- Preferred experience in clinical inpatient, outpatient, or community health settings
- Knowledge of the State’s selected standardized assessment tool, such as the CANS and CANS Decision Support Matrix
