Requirements

• Master's degree in social work, human behavioral sciences field, or another healthcare field
• 3 years of clinical, care management, and/or healthcare experience
• Ability to perform clinical assessments and develop clinical care plans
• Knowledge of clinical care coordination and utilization management
• Ability to establish and maintain connections with members of diverse populations
