The Case Manager will work collaboratively with case managers, supervisors, and community-based partners to promote long-term solutions for families, children, and individuals experiencing homelessness or who have formerly experienced homelessness. The role will involve developing and implementing care plans, coordinating services and resources, monitoring client progress, and providing advocacy and support.
