Job Description
The Case Manager role at the Colorado Coalition for the Homeless involves developing and implementing care plans, coordinating services and resources, and providing advocacy and support to individuals experiencing homelessness or who have formerly experienced homelessness.
Requirements
- Valid driver's license
- High School Diploma or GED
- Bachelor's degree in Psychology, Social Work, Human Services, or Criminal Justice preferred
- 1-2 years of experience working with a diverse population who are experiencing mental health challenges, particularly those individuals diagnosed with serious and persistent mental illness (SPMI) preferred
- Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred
