Job Description
The Clinical Patient Navigator provides patient navigation services to clients seeking services through the Colorado Health Network Medical Clinic, promoting access to medical care and addressing barriers to treatment. The role requires a bachelor's or master's degree in a related field, 1-3 years of experience in healthcare, and proficiency in software such as CAREWare and ARIES. Bilingual Spanish skills are desired.
Requirements
- Bachelors or Masters Degree in nursing, social work, sociology, psychology, counseling, or public health
- 1-3 years of experience working in a healthcare setting
- Experience using CAREWare, ARIES, and EMR preferred
- Experience working in a clinical setting
