As a Career Services Case Manager, you will work with individuals, families, and other departments to ensure services meet individual needs and promote choices. You will build and foster professional relationships, maintain confidential files, and complete necessary documentation.

Requirements

• Build and Foster professional relationships with individuals on her/his caseload, family members, and members of the ISP team
• Maintain routine and effective communication with members of the ISP team regarding progress, service planning, and incident reporting
• Schedule meetings routinely to discuss individuals’ progress and/or needed support areas
• Ensure that all necessary documentation is completed in a timely manner
• Actively provide input and coordinate with other departments and external referral sources
