Job Description
As a Day Program Case Manager, you will build and foster professional relationships with individuals with developmental disabilities and their interdisciplinary teams to ensure ongoing coordination of services. You will also coordinate and complete assessments, monitor support plans, and provide personal care and assistance to individuals. Qualifications include a high school diploma (BA degree in Human Services or related field preferred), at least 1 year of experience supporting adults with developmental disabilities, and a valid driver's license.
Requirements
- High School diploma required (BA degree in Human Services or related field preferred)
- At least 1 year of experience supporting adults with developmental disabilities is preferred
- A passion and dedication to supporting our people is a must
- Valid Driver’s license, reliable transportation, and acceptable driving record
- Must pass company background and reference checks
