Day Program Case Manager

The Day Program Case Manager will work to build and foster professional relationships with Individuals with developmental disabilities served on his or her caseload and members of each Individuals interdisciplinary team to ensure ongoing coordination of services.

Requirements

• High School diploma required (BA degree in Human Services or related field preferred)
• At least 1 year of experience supporting adults with developmental disabilities is preferred
• Valid Driver's license, reliable transportation, and acceptable driving record
• Must pass company background and reference checks
