Requirements

• Manage an assigned caseload of program members in collaboration with an RN Care Manager
• Complete comprehensive member assessments, member centered care plans, & functional screens
• Provide ongoing care coordination and quality assurance monitoring of services
• Maintain required documentation in adherence to established standards
