Community Options, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver care to individuals with disabilities in a community setting. The LPN will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
Requirements
- Provide treatment and health care procedures ordered by a physician
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
- Advise supervisor of all concerns and issues related to the provision of nursing services
- Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team
- Train employees on the medical needs of assigned individuals
- Monitor health and medical conditions of individuals and refer for medical services as needed
- Monitor the use of medications through medication administration logs
- Ensure health service supplies are available and replenished as needed
- Ensure program documentation and billable records are completed accurately and timely
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
- Participate with individuals in community, social, and recreational activities
- Participate in circles of support for personal futures planning
- Follow strict OSHA procedures regarding blood borne pathogens
