Senior Community Integration Counselor

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are Now Hiring a Senior Community Integration Counselor to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities.

Requirements

• Assist individuals with daily living skills and the development of community involvement, communication skills, problem-solving skills, and social skills
• Provide ongoing support for community integration efforts
• Administer medications and ensure proper completion of necessary documentation
• Plan, develop, facilitate, and participate in individual and group activities with participants
• Develop individual day program support plans
• Provide reports on individual progress in relation to overall goals
• Maintain inventory and ensures adequate supplies for program
• Oversee consumer funds as needed
• Monitor the health and medical needs of individuals and immediately report any concerns
• Drive individuals to medical appointments and activities in the community
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
