The Rapid Re-Housing Case Manager supports young adults in securing and maintaining permanent housing, using a trauma-informed, client-centered approach, and helping youth develop essential life skills, connect with employment and education opportunities, and access supportive services that promote long-term housing stability.
Requirements
- Conduct client intakes, assessments, and develop individualized housing and service plans.
- Assist clients with locating safe and affordable housing; support lease negotiations and move-in coordination.
- Provide consistent case management services, including home visits, goal setting, budgeting, and crisis intervention.
- Collaborate with landlords, property managers, and community partners to support successful tenancy.
- Maintain accurate and timely documentation in HMIS and internal databases.
- Coordinate access to resources including mental health services, employment programs, benefits, and transportation.
- Work in partnership with the Rapid Re-Housing Director and team to meet program goals and comply with funding requirements.
- Attend staff meetings, trainings, and community provider meetings as needed.
